Start with a large black canvas, in your layers window make a new layer set. Name it Steel Door.
Choose your rounded rectangle tool and set the radius setting to 10px and check anti-aliased. In a new layer draw a 121 x 119 pixels, #202020 rounded rectangle. Use the fixed size setting under rounded rectangle options to make the exact size.

Cut out the shape you seen on the picture shown below.
Add the following layer styles:

Now uplicate the shape layer from step two. Instead of re-creating the entire effect, just make a new layer, leave this empty. Merge your empty layer with the duplicate.
Flip the new shape layer horizontal and position away from the original.

Now choose your roundeded rectangle tool and set the radius setting to 40 pixels and check anti-aliased. In a new layer draw a 112 x 52 pixels, white rounded rectangle.
Set the layers blending mode to Darken and add the following layer styles:

Again choose your rounded rectangle tool and set the radius setting to 8 pixels with anti-aliased checked. In a new layer draw a 119 x 78 pixels, #202020 rounded rectangle.
Cut the bottom where the curves meet and add the following layer styles:

Duplicate your rounded rectangle structure layer from step four. Cut half of the shape.
Set your layers blending mode to Darken and add the following layer style:

Now in your layers window select/highlight your Steel Door layer set. Then above your screen select Layer then Merge Layer set. You should end up with your steel door in one layer.

Finish it off with some textures, with your steel door layer dd a pattern overlay layer style:

Steel Door.
